Cyclophosphamide 1–2 mg/kg per day (maximum dose = 150 mg/day), orally in the morning, continually if a response occurs (total dosage/week = 7–14 mg/kg; maximum dose/week = 1050 mg)
Encourage patients to increase their consumption of nonalcoholic fluid (64–80 ounces [1900–2400 mL]) to prevent developing hemorrhagic cystitis
or
Azathioprine 1–2 mg/kg per day (maximum dose = 150 mg/day), orally, continually if a response occurs (total dosage/week = 7–14 mg/kg; maximum dose/week = 1050 mg)
or
Mycophenolate mofetil 750–1000 mg/dose orally twice daily for a minimum of 12 weeks (total dose/day = 1500–2000 mg)
Supportive Care
Antiemetic prophylaxis
Emetogenic potential with cyclophosphamide is LOW
Emetogenic potential with azathioprine and mycophenolate mofetil is MINIMAL–LOW

Hematopoietic growth factor (CSF) prophylaxis
Primary prophylaxis is NOT indicated

Antimicrobial prophylaxis
Risk of fever and neutropenia is LOW

Therapy Monitoring
Monitor CBC and leukocyte differential serially—initially at least weekly, then every other week
Indications
Patients with persistent thrombocytopenia despite prior therapy with glucocorticoids and a prior splenectomy
