This textbook is dedicated to the memory of Professor Daniel E. Koshland, Jr, who instilled in me the love of science; to the memory of Dr. Luther W. Brady, MD, who inspired me to master clinical oncology and to appreciate the art of humanity; and Professor Steve P. Calandrillo, my law school professor, who takes every opportunity to support and encourage me to explore the intersection of law and medicine. Most important of all, I dedicate this textbook to my patients and US Veterans, who have taught me so much all these years so that I can use the knowledge gained to improve the delivery of cancer care that is high quality, equitable, and provided with compassion.
